Disclosure Request Procedure

1. Disclosure Requests

In accordance with the Personal Information Protection Act, an Individual Concerned (or their representative) may request disclosure of personal data we retain on the Individual Concerned in accordance with the following procedures; provided, however, that, we will not be able to respond to all or parts of a request in the following situations:

  • ・When there is a possibility of harming the Individual Concerned or a third party’s life, body, property or other rights and interests
  • ・When there is a possibility of significantly hindering the proper implementation of our business
  • ・When disclosure will violate laws and regulations

2. Request Procedures

(1) Contact Point for Accepting Requests

Please contact the following office by telephone or in writing to request an application form for disclosure. Forms will be sent by post.

(2) Submitting an Application Form

Please contact the following office by telephone or in writing to request an application form for disclosure. Forms will be sent by post.

Documents to be Submitted

  • 1. Application form: please fill in all necessary details
  • 2. Identity verification document (one copy)
    To prevent personal information being divulged to third parties other than the Individual Concerned, we request submission of a copy of one of the following documents to verify your identity:

a. Health insurance card
b. Pension book
c. Disability certificate
d. Driver’s license
e. Passport
f. Alien registration certificate
g. Copy of certificate of residence
h. Certificate of registered matters on alien registration certificate

  • * a, b, c, d, e, and f must have not expired or must be currently valid, g and h must have been issued within six months of the date submitted. We cannot accept documents that do not include your name, address, or date of birth.
  • * We will destroy the above-stated documents after a certain period of time.

3. Other Matters
If a request is being made by a representative, please send the following documents in addition to (1) and (2) above.

  • i. Representative’s identity verification document (one copy)
    Copy of one of the documents listed in (2) above
  • ii. Document that proves the relationship to the Individual Concerned (one copy)
    Parental authority: certified copy of family register
    Guardian of adult: certificate of registered information on guardian of adult
    Mandatary: Power of Attorney and Individual Concerned’s registered seal certificate
  • * Documents must be originals issued within six months of the date submitted.

Fees

  • ・The following fees are required when submitting an application form.
    500 yen per application (including consumption tax)
  • ・Cash sent by registered post or 500 yen stamp

3. Notification of Outcome of Investigations

We will notify you by post of the outcome of our investigation based on your request and the details of any personal data we retain.

Suspension of Use and Correction, etc. Request Procedure

In accordance with the Personal Information Protection Act, an Individual Concerned (or their representative) may request suspension of use, etc. or suspension of provision to third parties of personal data we retain on the Individual Concerned; provided, however, that, in accordance with laws and regulations, we may not be able to respond to certain requests.

2. Request Procedures
